Complete Registration Instructions

To obtain 5 CME credits for having studied this tutorial, take the following steps:
  1. Study the whole tutorial (about 5 hours)
  2. Using the print button of your browser, print this page to obtain a paper copy of the registration form. Fill it out.
  3. Go to the page with the evaluation form, use the print button of your browser to print it out, and complete it.
  4. Go the page with links to the CME quizzes and decide which CME quiz is best for you (with or without morphology-based questions). Proceed to that CME quiz page and use the print button of your browser to print out a paper copy of the quiz.
  5. Take the CME quiz, marking your answers on the paper copy.
  6. Mail the completed registration form, evaluation form and CME quiz along with $75 to the address above.
