บรรยายพยาธิสภาพ gross ของรอยโรคจากภาพ (3 คะแนน) Lung measures 13 X 7 cm. in dimension. Cut surface is solid and dark red. There are a few small areas of yellowish white discoloration, ranging from 1 to 2.5 cm. in diameter
ปอดมีขนาด 13 x 7 ซม. หน้าตัดทึบและสีดำแดง พบมีรอยโรคสีเหลืองขาว ขนาดเส้นผ่าศูนย์กลาง 1 ถึง 2.5 ซม.

บรรยายพยาธิสภาพ จากกล้องจุลทรรศน์ ของรอยโรค (3 คะแนน) There are diffuse vascular congestion and edema. with widespread infiltration of acute inflammatory cell, especially numerous polymorphonuclear leukocytes in the alveolar parenchyma. Some areas display accumulation of polymorphonuclear cell filling the alveolar space. Some alveolar septum are torn away or destroyed and create a larger space forming abscesses.
พบเส้นเลือดขยายตัวและเม็ดเลือดแดงสะสมเต็มในหลอดเลือด (vascular congestion) พร้อมกับของเหลวสีชมพูแดงสะสมในช่องว่าง alveoli ของปอด ภายในช่องว่างเหล่านี้ พบเม็ดโลหิตขาวชนิด polymorphonuclear cell ออกมาสะสมเป็นจำนวนมาก บางแห่งของเนื้อปอด พบ เม็ดโลหิตขาวชนิด polymorphonuclear cell บรรจุเต็มในช่องว่างทำให้ช่องว่างขยายตัวออก จนผนังถุงลมปอดแตก หรือถูกทำลาย เป็นช่องว่างที่ใหญ่ขึ้น กลายเป็นหนองฝี (abscess)

วินิจฉัยรอยโรค (Diagnosis) (1 คะแนน) Bronchopneumonia with abscesses.
	หรือ Pneumonia with abscess.
	หรือ ปอดอักเสบเป็นหนองฝี.

เหตุผลในการวินิจฉัย (1 คะแนน) : There are vascular congestion and edema of alveolar parenchyma together with massive accumulation of acute inflammatory cell (PMN leukocyte) and tissue necrosis forming areas of abscess.
เนื่องจากมีการเปลี่ยนปลงของเส้นเลือด และบวมน้ำ พร้อมกับพบเม็ดเลือดขาวสะสมเป็นจำนวนมาก ทำให้มีการทำลายถุงลม จนกลายเป็นหนอง
